Outreach Worker needed to actively seek out and engage with adults with mental health problems who have high support needs and experience isolation, in order to assist them in accessing specialist and mainstream services. You will be supporting people with mental health problems in making informed choices about, and making use of, specialist and mainstream community services, resources and facilities. You will negotiate opportunities and develop access routes into a range of activities in the community in order that adults with mental health problems can participate in community life. You will deliver high quality and effective person centred planning for adults with mental health problems. You will develop expertise and local knowledge in one or more life domains as agreed at job commencement, or as directed by the line manager. You will also carry out duties in a manner, which actively demonstrates commitment to the organisational objectives as they relate to individual posts and in particular to be pro-active in working in a socially inclusive way. You must be educated to a degree level (preferably in psychology, social care or education) or its equivalent. You must hold a relevant professional qualification (social work, psychology, nursing, occupational therapy). You must have a NVQ Level 3 in Care (or working towards it). It is essential you have 2 or more years’ experience (in paid or voluntary capacity) of working with people with severe mental health problems. |
