We are currently seeking an Assistant Care Manager to contribute towards the maintenance of high quality services and the promotion of independence of users and carers by reviewing domiciliary care packages and/or Residential and Nursing Home placements.

You will carry a caseload of service users who require visiting in order to carry our reviews/re-assessments of the services they are receiving. which have been arranged by and/or are funded by Merton Social Services. You will undertake reviews and standard assessments of need and identify appropriate provision of services, domiciliary services, day and residential care, both in the local authority and voluntary sector.
